The
following checklist is meant to help guide you through the
critical paperwork that is required of new employees, and
also to help orient you to all of the opportunities and benefits
available to you as an employee of the University of Maryland.
Complete
paperwork required for entry into payroll system. Those
items that are hyperlinked are available for you to download
and print out. There are a few documents that are three
part carbon forms and must be obtained from Pauline Rirksopa
in room 1101, Physics.
When
these items are complete, return to Pauline Rirksopa, Payroll,
Room 1101, Physics.
Once this form is completed and your personal information
appears in the university system, you can update your
home and campus addresses, phone numbers, and email address
on the Web. The URL is http://www.inform.umd.edu/About/howtofix/ This
is the same information published in the Faculty/Staff Directory,
both in print and on the Web.
You can also change the name, address, and phone information
of the person you have listed as your Emergency Contact, as
well as several other items of a personal or demographic nature.
Staff ID enrollment form (complete, obtain signature from
Andrea Pope in room 1105 and take directly to the Mitchell
Building to receive your picture ID)
If you are employed at 50% or more
as a permanent employee (6+ months) and have a faculty or
staff status, you must enroll in a retirement plan as a condition
of employment.
If you are non-exempt staff, you must enroll in the
Maryland State Pension System.
If you are exempt staff or faculty, you may choose either
the pension system or one of the following plans: TIAA-CREF,
FIDELITY, or VALIC.
In order to receive a paycheck you must complete the
following paperwork within 14 days of date of hire and return
to Andrea in room 1105:

reduction Agreement
Election
not to participate in the teachers system form (*only
if you chose TIAA-Cref, Fidelity or VALIC)
Elect
health benefits
If you wish to obtain health insurance
as part of your employment at the University, you must submit
a year 2 enrollment form within 60 days of date of hire.

Complete
a year 2006 enrollment form indicating your selections.
If requesting coverage for dependents, provide appropriate
documentation of relationship (birth or marriage certificate).
Submit
completed form to Andrea Pope, Room 1105, Physics, or
Lorraine DeSalvo, Room 1109, Physics.
Obtain
access to workspaces (offices, labs, etc.)
See
Andrea Pope, Room 1105, Physics, to receive keys/access
to your workspace, and any other access that may be required.
If you are a
graduate student, you must also obtain a building pass from
Andrea.
Obtain
parking permit and review parking procedures
If
you plan to park on campus, you must complete a parking
registration form. If you receive a biweekly paycheck,
then you are eligible for payroll deduction. Comple
the payroll deduction card (available in Room 1105, Physics)
if you wish to have the biweekly fee deducted from your
paycheck automatically. If
you are not eligible for payroll deduction, you may pay the
yearly parking fee by check or credit card. When
complete, return these to Andrea Commock-Pope, Room 1105, Physics,
for her signature. These
forms can then be brought to the Campus Parking office located
under the Regents Drive Garage. Parking fees can be found on the Campus Parking Website.

Apply
for a computer account
Obtain
a user ID for appropriate applications, create an email account.
You may apply for an account on http://internal.physics.umd.edu/pnce Follow the instructions carefully when choosing your

Hire Orientation class held by the Training and Development
Office
of the Personnel Services Department.This class will supplement the information provided
here as well as provide more detail on the benefits available
to you as a new employee of the University of Maryland,
College Park. These
